Vault prolapse – the descent of the vaginal apex – is a challenging condition that occurs when the supportive structures of the pelvic floor are weakened, most commonly following hysterectomy, but also due to other risk factors such as vaginal delivery and ageing. It predominantly affects postmenopausal women and left unaddressed, can significantly undermine quality of life, causing pelvic pressure, urinary dysfunction and sexual impairment.
Managing vault prolapse requires careful surgical decision-making, with multiple effective procedures available but no single approach suited to every patient. Understanding the indications, technical considerations and outcomes of each technique is key to delivering patient-centred care that improves symptoms, restores function and enhances quality of life.
This webinar, led by the FIGO Committee on Urogynaecology and Pelvic Floor Disorders in collaboration with the International Urogynecological Association (IUGA), brought the full spectrum of surgical approaches to vault prolapse into focus. Expert speakers showcased abdominal sacral colpopexy, uterosacral ligament plication, sacrospinous fixation, and colpocleisis. Covering abdominal, endoscopic and vaginal repair options, they shared practical tips and tricks drawn from extensive clinical experience.
